Re: Change Order #4 — 2010 Infrastructure Improvements
Council Action Requested:
Attached for Council approval is Change Order 44 for the 2010 Infrastructure Improvements.
This change order was assembled to account for several work items that required in -field changes
with storm sewer system components and thus were either beyond the project scope or due to
unforeseen circumstances. Also included is compensation to the contractor for providing
additional finish grading associated with the replacement of failed base materials on parts of
Westview Drive.
Background Information:
Please see the attached change order for further description of the work items.
Financial Impact:
Thus far, the estimated total impact of change orders on this project amount to a reduction of
$3,967.10, or about 0.20% of the total contract value.

Contract Name: 2010 infrastructure Improvements
The City of Hastings and Hardrives. Inc. hereby agree to this change order to compensate for adjustments equired to install
several storm sewer pipes and structures.
Field Relocation of Storm Structure - The contractor was ordered to install a storm structure at a rev sed location on
Westview Drive clue to a, conflict with an existing gas main. Field modification of the structure was also required to
meet the grade requirements at the new location. The cost of the work is hereby agreed to be $8,67 4.60, and a new
item in the account records will be created to compensate for this work. It will be described as: CH %NGE ORDER
#4 — FIELD RELOCATION OF STORM STRUCTiJRE, Please see the table on the reverse -ide for further
information,
Field Modification of Storm Pipe Due to Utility Conflict — The contractor was required to remove a) existing stone;
pipe that had a Comcast Utility cable bored through the pipe on Westview Drive. Payment s for the time,
equipment and materials required to remove the pipe and to modify the location of the cable to allow for installation
of the new storm pipe. The cost of the work is hereby agreed to be $4,136.00, and a new item in the account records
will be created to compensate for this work, it will be described as: CHANGE ORDI1 t14 .. FIELD
MODiF.ICATION Of STORM PIPE: DUE TO UTILITY CONFLICT. Please see the table on the reverse side for

Field Modification of Storm Pipe and Structures Due to Sanitary Sewer Conflict, — The contractor was required to
modify the grade of an installed pipe to avoid a conflict with an existing sanitary sewer main on estview Drive.
Field modification of 2 structures was also .required to meet the revised pipe grade, The cost of the work is .hereby
agreed to be 52,138,40, and a new item in the account records will be created to compensate for this work, It will be
described as: CHANGE ORDER 44 — FIELD MODIFICATION OI" STORM PiPE AND STRUCI URFS DUE TO
SANITARY SEWER CONFLICT. Please see the table on the reverse side for further information.
4, Repair of Existing Storm Sewer Pipe — 'I1le contractor was requested to repair an existing sLorn sewer pipe on
Westview Drive that had become misaligned and bad subsequently caused a sink hole in the existin street adjacent
to the project limits. Payment is for the time, equipment and materials required to make the necess try repairs. The
cost of the work is hereby agreed to be $3,869.50,. and a new item in the account records w' 1 be created to
